By submitting this application you are certifying that you are an authorized user on this credit card and are authorized to enter into this billing agreement for this card. Furthermore you are authorizing us to charge your credit card for monthly service fees associated with this agreement.

By submitting this application, you authorize BATS to check the validity of information provided herein, including, but not limited to, corporate information, MC#, insurance, etc.

Subscription to BATS software is a monthly subscription that is renewed automatically at the beginning of each billing cycle. This is commonly referred to as a “Month To Month” agreement. Both parties can cancel this agreement at any time by written notice. Cancellation will be effective at the end of the then current billing cycle. Partial refunds of monthly fees are not allowed. Monthly fee is calculated based on the number of users who have logged into the software in the previous monthly billing period, but the payment that is applied to the account covers the license fees for the current monthly billing cycle. Minimum monthly charge is $200/month even if no users have logged in during the previous billing cycle.
